First Time Registration

Please complete the Registration Forms in the “Medical and 咨询 Clearances” 的部分 Student Wellness Center Patient Portal. 

  • Registration forms for counseling 任命s will be provided 一个 hour before the 安排约会. Additional forms will be completed during check-in.
  • For health or wellness 任命s, please complete registration forms as soon as 可能的

Are you a minor (under 18 years of age)?

Minors must complete the Authorization for Treatment of a Minor form prior to receiving Student Wellness Center services.

In California, minors are individuals under 18 years of age. California Family Code 6900及以下顺序. states that unless a specific exception applies, a minor’s parent or legal guardian must consent to medical & 精神卫生保健. Exceptions in which the minor does not need parent/guardian consent are listed in the form.

If your 任命 is in-person, please go to the appropriate check-in location indicated in your 任命 information at the Student Wellness Center unless otherwise instructed. 请把 你的学生证.

  • Arrive 15 minutes before your 任命 time and expect to be at the Student Wellness Center for approximately 60 minutes.
  • For laboratory 任命s (ex: UTI, pregnancy tests, etc), you will be instructed on when to arrive for your 任命

咨询预约

If your 任命 is in-person, please go to the Student Wellness Center and check-in at 咨询 and 心理服务 located in 300B (3rd floor). 请把 你的学生证. Arrive 15 minutes before your 任命 time (unless specified) at the CAPS front desk on the 3rd floor

Telehealth (Zoom) Appointment 

If your 任命 is a Telehealth/Telemedicine 任命, you will need to check in to your 任命 by logging in and clicking “Appointments” in the Student Wellness Center Patient Portal (via computer, ph一个, or tablet). Please do not go to the Student Wellness Center.

  • For an initial consultation 任命, log on 30 minutes prior to your scheduled online (zoom) 任命
  • For an intake 任命, check-in 20 minutes prior to your scheduled online (zoom) 任命. Complete forms in the “Survey Forms” section in the Student Wellness Center Patient Portal.
